EVERETT, Wash. - The state Department of Labor and Industries has fined Goodwill $9,000 for workplace safety violations in a forklift accident that killed a man in August in Lynnwood.
The department says Goodwill should have done a better job training the forklift operator.
The Daily Herald reports the state agency released its findings Wednesday.
The forklift driver told officers he saw a "flash of blue" go in front of the forklift and he slammed on the brakes. The box on the forklift then fell forward and landed on 69-year-old Chuck K. Lee of Bothell.
"It appears that the forklift driver and the victim - neither one of them saw each other," said Det. Brian Jorgensen with Lynnwood Police. "It was a big box, but it wasn't lifted very high so whether he could see over or around it. I don't know at this point."
Lee was rushed to Harborview Medical Center in Seattle in grave condition and later passed away, a hospital spokesperson said.
